Chronic Disease Management
The Chronic Disease Management Program supports individuals living with long-term conditions such as diabetes, heart disease, and respiratory illness.
Our aim is to:
- Improve quality of life
- Promote healthy ageing
- Empower people to manage their own health
This is achieved through health education, regular assessments, personalised care planning and monitoring by our multidisciplinary team.
Rheumatic Heart Disease (RHD) & Acute Rheumatic Fever (ARF)
Acute Rheumatic Fever (ARF) and Rheumatic Heart Disease (RHD) are preventable but serious conditions that continue to affect Aboriginal communities at high rates. These diseases often begin with untreated Group A Streptococcus infections of the throat or skin, which can lead to permanent heart damage if not addressed early.
Our health promotion team works to:
- Increase awareness of ARF and RHD
- Promote early diagnosis and treatment
- Support regular follow-up care for those affected
- Reduce new infections and long-term complications
We are committed to supporting individuals and families through education, clinical support, and community outreach.
Sexual Health – Syphilis Enhanced Response
In response to the ongoing syphilis outbreak and high levels of other STIs and BBVs (Blood Borne Viruses), YYMS is delivering targeted sexual health education, testing, and support.
The goals of this program are to:
- Increase STI and BBV testing in the Halls Creek region
- Reduce transmission of infectious syphilis and related infections
- Promote safer sexual health practices
- Align with the National BBV and STI Strategies (2024–2030)
Our approach includes opportunistic testing, health education, and long-term strategies to build awareness and reduce stigma around sexual health.
School Health (Ringer Soak)
In partnership with WACHS, YYMS provides school-based health services in the Kundat Djaru (Ringer Soak) community. The program includes:
- General health checks
- Health and hygiene education
- Support for growth, nutrition, and development
- Promotion of healthy lifestyle choices among school-aged children
Tackling Indigenous Smoking (TIS)
The Tackling Indigenous Smoking (TIS) Program is focused on reducing smoking and vaping-related harm in Aboriginal and Torres Strait Islander communities. Through a culturally safe, community-led approach, our program works with individuals, families, and communities to promote healthier lifestyles and support people to quit smoking or vaping.
Our team delivers education, support, and community activities that empower people with the knowledge and tools to improve their health and wellbeing.

Our Program Aims
The Tackling Indigenous Smoking Program focuses on three key areas to support healthier communities.
Prevent Smoking and Vaping Uptake
We work to increase community awareness about the harms of tobacco and vape use and the benefits of remaining smoke and vape free. Through education and community engagement, we aim to reduce the number of people who start smoking or vaping.
Support People to Quit
We support community members who want to quit smoking or vaping by increasing awareness about the benefits of quitting and encouraging people to take positive steps towards cessation. Our team works with health professionals to connect individuals with the support and resources they need.
Reduce Exposure to Second-Hand Smoke and Vape Aerosol
We promote smoke and vape-free environments to protect families and communities from harmful exposure. This includes encouraging smoke-free homes, cars, workplaces, and community spaces and raising awareness about the risks of second-hand and third-hand smoke.
